Ladies Handbags with Buckles Containing Co-60 - 08 May 07
"On 21 December 2006 the Inspectorate was notified by the Customs Office at Rotterdam Maasvlakte that they had a radiation alarm of their portal detector. Using handheld survey equipment an enhanced radiation level at the surface of a 20 ft container of 1.5 microSieverts per hour could be determined.
The container was completely filled with several types of bags, most of them ladies handbags."
Potential Overexposures to Members of the Public - 02 Mar 07
"On 22 February 2007, at approximately 9.00 am, an individual working for the licensee noticed what appeared to be a component part laying on the ground at the licensee’s facility. The individual did not know the component was radioactive, so he picked it up and put it in his jacket pocket. Additionally, the individual visited several businesses and hung his jacket in an employee break room for the remainder of the day and overnight.
On 23 February 2007, at approximately 6.00 am, the well logging crew unpacked their equipment and realized the source was missing. They immediately initiated a search and retrieved the source from the individual . . ."
